In turn, we encourage our patients to maintain consistent communication with our staff about any questions, changes in symptoms, updates on insurance, or adjustments needed in appointment times or dates. To schedule an appointment please call our main scheduling number at 877.415.4116. Through our centralized scheduling capabilities, our appointment representatives are available to schedule an appointment at any of our locations most convenient to you. This scheduling may be done Monday – Thursday from 8 a.m. to 4:30 p.m and Friday from 8 a.m. to noon and from 12:30 p.m. until 4:00 p.m. Voicemail prompts are available outside office hours. Feel free to leave a message and a representative will return your call. For your convenience, you will receive an appointment reminder 48 hours before your scheduled appointment by email, text or phone. When calling, please be prepared to provide or confirm the following:

  • Date of birth
  • Brief description of the reason for the appointment
  • Address
  • Home and alternate phone numbers
  • Health insurance information
  • Name of referring physician
  • Referring physician order/referral to ensure the scheduling of the appropriate appointment or test

To Reschedule or Cancel an Appointment:

Please call our main scheduling number, 877.415.4116, to reschedule or cancel an appointment. Your appointment time is reserved exclusively for you. A 24-hour notice is required for cancellation or rescheduling of appointments to avoid a missed appointment fee.

Preparing for Your Visit

We welcome you as a new patient to Carient. To help ensure your first visit is smooth and worry-free, we have provided some useful information and recommendations below.

In order to help you prepare for your first office visit, here is a list of items you will want to bring along:

  • Insurance card(s)
  • Photo identification, such as a driver’s license
  • Referral form (for HMO insurance, if required)
  • List of current medications you are taking including prescriptions, over-the-counter medications and any herbs.
  • Relevant information about your medical and surgical history. Bring any pertinent x-rays or records you may have.
  • Important questions you would like to ask the doctor.

Having these items ready will help us serve you more efficiently and ensure that all of your concerns are addressed at your appointment.

Patient Forms

We encourage you to request an invitation for our patient portal prior to your first visit. There, you will have access to new patient forms as well as the ability to make appointment requests, ask your physician/nurse a clinical question, pay your bill online and request medical records. We encourage you to sign up for the portal 3 days before to your first visit to streamline your appointment. If you’d like to fill out a paper copy of your new patient forms, click here if you are:

  • A new patient, or
  • A former patient who has not been seen by one of our doctors in the past two years.

Once you have downloaded the form please print it and bring the completed form with you on the day of your visit.
